Medicaid Service Coordinators (MSC)
Unique People Services’ Medicaid Service Coordinators (MSC) provides ongoing support to individuals with developmental disabilities and their families. We provide assistance with obtaining available services in the community as well as services under the Home and Community Based (HCBS) Waiver. In addition, we also assist people with going through the eligibility determination process with the New York State Office for People with Developmental Disabilities (OPWDD).
Currently Unique People Services Medicaid Service Coordination program serves individuals over 21 only. Our Service coordinators work together with families to develop and maintain a personalized plan designed to meet each person’s individual needs and unique goals. We provide assistance with finding a day program, residential placement, financial supports, recreational activities, respite, medical providers, etc. Our Medicaid Service Coordinators actively advocate to protect the rights of people with developmental disabilities. This includes attending and advocating at ISP meetings and meeting face-to-face with the persons on their caseload as often as needed, but at least three times during the year. All this is done with the utmost professionalism and compassion.
